How to Apply for a Grant

  • Name of Organization to which grant would be paid. Please list exact legal name.

Background of the Organization (2 page maximum)

  • A brief description of your history and mission
  • The primary need or problem that you work to address including: target population, geographic location and socio-economic status
  • Current programs and accomplishments
  • Your relationships --- both formal and informal --- with other organizations working to meet the same needs or providing similar services. How does your program differ from other existing programs?
  • Number of paid full time staff, paid part time staff and number of volunteers

Funding Request: Project Description (2 page maximum)

  • How the project contributes to your organization’s overall mission
  • The population you plan to serve
  • Anticipated length of the project
  • The project goals
  • The project activities you will use to implement the project goals
  • The anticipated changes in behavior, skills and knowledge of the program  participants
  • Names, titles, e-mails and brief background qualifications of individuals who will direct the project
  • If the funding request is for a scholarship grant for an individual who will be working within the project, describe their background qualifications and criteria for their acceptance to your program

Evaluation (1 page maximum)

  • How will the program be monitored and evaluated for its effectiveness?
  • How will your program's outcomes be communicated to the public?

Attachments - For this project

  • A current expense budget for the project. Indicate specific uses of the requested grant.
  • A list of all sources of income toward the project, actual and perspective with amounts

Attachments - From your organization

  • A copy of your most recent IRS letter indicating your organization’s tax exempt status
  • Most recent financial statement, audited if available
  • Operating expense budgets for the current and most recent fiscal year
  • A list of foundation and corporate supporters and all other sources of income, with amounts, for your current and most recent fiscal year
  • Foundations, corporations and other sources that you are soliciting for funding
  • A list of your Board of Directors with their affiliations
  • A copy of the most recent annual report, if available

Grant Application Dates
April 1   or   October 1
If your application is accepted, a site visitation or phone interview will then be scheduled by appointment. You will hear the foundation response on:
June 1   or   December 1


Distribution of Grant Funds
If your program is chosen, your organization will be placed on the funding timeline according to the start date of you project.

At this time we are accepting attachments saved in Microsoft Word (.doc,.rtf) and Adobe Acrobat (.pdf). Please do not send any other file formats and please limit attachments to 5mb in file size.
